Regarding when to make the b'rakha: one can say the b'rakha before (Rambam, Ramban et al) or after the n'ttila (Rav Sa'adhya Gaon, Tosaphoth et al). Both are correct. There is good reason to believe that the original practice was to recite the b'rakha for n'ttila and tt'vila in a miqwe AFTER the deed.
- Also I understood that Rambam differs as to the timing of making that bracha (before washing) as opposed to how its done by most today. How does the Rav say one does the Bracha (before or after), also what if one washes in the bathroom (at least in the morning)- do they do the bracha before going into the bathroom, or not?
